Structured Family Caregiving Waiver (SFCW)

The Structured Family Caregiving Waiver (SFCW) is a program designed to allow family members, including spouses, or close friends to care for loved ones at home who have Alzheimer’s disease or other dementia-related conditions. Similar to the Consumer Directed Services program but tailored specifically for families caring for individuals with active Missouri Medicaid and dementia, SFCW provides an opportunity for family members or household residents to offer care and companionship at home for the first time.

How the Program Works

The SFCW includes homemaker and attendant care services as outlined in the participant’s plan of care. These services cover tasks such as:

Assistance with activities of daily living (ADLs).
Medication management and oversight.
Escorting participants to medical or therapeutic appointments and community activities that help maintain natural supports.
Providing supportive and health-related services that compensate for physical or cognitive limitations.

Additionally, a qualified substitute caregiver, chosen by the participant and employed by Golden Heart Care Center, can provide services when the primary caregiver is unavailable.

Eligibility Criteria

Participants must meet the following criteria to qualify for SFCW:

Be 21 years or older.
Be enrolled in Missouri Medicaid with an appropriate Medicaid Eligibility (ME) code.
Have a diagnosis of Alzheimer’s disease or a related disorder as defined by 172.800 RSMo.
Have a primary caregiver residing in the same household.
Identify a qualified substitute caregiver (chosen by the participant or legal guardian).
Complete a comprehensive face-to-face assessment or reassessment with DSDS or its designee.
Meet Nursing Facility Level of Care (LOC) requirements.

Restrictions

Enrollment in the SFCW is subject to these restrictions:

Participants cannot be enrolled in any other state plan or waiver services, as SFCW is intended to address all care needs.
Participants cannot reside in any type of facility.
Only one primary caregiver may be hired.
