Healthy Children & Youth Program (HCY)

At Golden Heart Care Center, our mission is to support families and children with special health care needs. Through the Healthy Children and Youth (HCY) Program, we provide service coordination and authorization for medically necessary in-home care for MO HealthNet recipients from birth to age 21. Our compassionate care team conducts home assessments, coordinates services, and connects families to vital resources to help children remain safely in their homes with their loved ones.

 

Who We Serve

Golden Heart Care Center provides care for children and young adults with conditions such as:

• Neurological disorders
• Respiratory conditions
• Gastrointestinal disorders
• Brain injuries
• Quadriplegia or paraplegia

Flexible Care Options
Care is available from as little as four hours per day to 24-hour care, depending on the needs of the individual.

Eligibility Criteria

To qualify for the HCY Program at Golden Heart Care Center, participants must:

• Be a Missouri resident.
• Be 0 to 21 years old.
• Require medically necessary services.
• Be enrolled in MO HealthNet.
